Special Education Summer Series- Back To School Forms
This week in all about Back to School forms. If you do quick search there are hundreds of great examples and freebies for you to use. I send home the one my school requires and then a small packet for me that includes some information the school forms don’t.
I use this editable packet for Hailey Deloya at The Autism Tank. She’s got a really nice set of forms that are free and editable and she’s got a nice post on why she includes each form. I use the following forms from her packet:
*welcome page-I edit it some and change the school supply list some
I don’t use the community outing form because we need permission for each specific field trip or off campus outing so it wouldn’t do me any good but I’m totally jealous that she can take her students on weekly community trips.
I add of couple of extra forms to the packet.
*Donors Choose photo release– so when I start a project I’ve already got that part taken care of.
*life skills rating form– so I have a better idea of what skills my students really need to focus on. This is from Sasha at The Autism Helper
I take the rating form and put everyone’s answers on a spread sheet that way I can clearly see what skills most of my students have down and what skills we need to focus one. It makes planning life skills much easier.
I also include a *Contact The Teacher* information sheet that has my school contact information and links to our private class blog. It also has commonly needed district numbers like for the transportation department so they can leave in on the fridge and have an easy reference sheet.
